Listed below you will find two files we would like for you to fill out for us called a "Membership or Personal Profile" and a "Placing Membership on Sunday" form.  These allow us to have all of your vital information to enter into our Automated Church System (ACS) as well as allowing us to help find a place to help you get involved.  You can either print out the PDF file and fill it out and return it to Beverly Kellar, or you can fill it out electronically in Microdoft word and email it back to us at bkellar@northdavischurch.org
